In my three decades of practicing medicine, I’ve seen so many  patients with run-away, preventable disease whose illness got out of control because they did not got to a doctor earlier due to cost concerns. Is Your Asthma e-Asthma? New Medicine! GET YOUR EOSINOPHIL TEST! * e-Asthma is category of severe asthma * Patients with e-Asthma have difficulty getting asthma control with usual treatment * e-Asthma responds well to a new type of injected medicine * In such patients, this new medicine can decrease their asthma attacks and allow them to use less oral steroid.      e-Asthma is a type of severe asthma that responds well to a new kind of control medicine that you inject under skin, usually monthly.      Severe Asthma means: You’ve been on a high dose of inhaled corticosteroid (ICS) and a long-acting bronchodilator or theophylline for in the past year; and/or You’ve been treated with steroids (like Prednisone) by mouth for more than half of the past year. Wondering what “high dose” ICS means?   Definition of high dose* ICS Age 6 – 12 years Age > 12 yrs Beclomethasone (Qvar) ≥ 320 mcg/day ≥ 1000 mcg/day Budesonide (Pulmicort) ≥ 800 mcg/day ≥ 1600 mcg/day Ciclesonide (Alvesco) ≥ 160 mcg/day ≥ 320 mcg/day Fluticasone (Flovent) ≥ 500 mcg/day ≥ 1000 mcg/day Mometasone (Asmanex)  ≥ 500 mcg/day ≥ 800 mcg/day *European Respiratory Society: Eur Respir J Feb 2014 43:343-373 This new type of injectable asthma medicine has been shown to dramatically: 1. Reduce the number of asthma attacks you will have 2. Reduce how much oral steroid (like Prednisone) and ICS you need to stay controlled. How do we know if your asthma is the type that will respond to this medicine?  You would be likely to get a good response to this new medicine if: Your eosinophils are elevated in your bloodstream, and Your allergy testing shows that you are triggered by seasonal allergens. So, let’s review: If  You have been on high dose ICS (see above) or oral steroid for more than half of the year, AND You have skin allergy tests that show a problem THEN I would recommend having your blood drawn to measure your eosinophil count. If your eosinophil count is elevated (35-700 IU/mL), you would be a good candidate to try this new type of medicine. What medications would be considered? Here is a list of subcutaneous medications that can be ordered for e-Asthma:   Omallzumab (Xolair) injected every 2 or 4 weeks depending on your eosinophil count and age Dupilumab (Dupixent) injected every 2 weeks Mepolizumab (Nucala) injected every 4 weeks Benralizumab (Fasenra) injected every 4 weeks for 3 doses, then every 8 weeks. These medications are quite new and it will take some time for general practitioners to become educated about prescribing them. But, they are available RIGHT NOW and if you are suffering from severe asthma, they could be a really good solution for getting your severe asthma under control. Follow-up Care * Follow-up Care is Essential to good asthma control. * Follow-up allows determination of asthma severity. * Other disorders that affect your breathing are detected early. * Rescue medication refills are provided * Your Asthma Action Plan is updated. Having your asthma diagnosed is the first step to getting control. But after treatments are prescribed, follow-up becomes the key to avoiding asthma attacks and minimizing your asthma symptoms Follow-up is so important because asthma is not a disease that is ‘cured’ in the usual sense. Instead, our goal is to manage it. Control it. So that you can be active. So you can sleep well. So you don’t have to make emergency room visits “routine”. So, you don’t have to experience life-threatening asthma attacks! Managing asthma means regular, repeated Dr. visits. We know from research studies that the best interval for these reassessments is about every 3 months. At these appointments: We work on a number of points. .. First, I evaluate your asthma severity to see if it has changed., .. We discuss your response to your asthma treatment plan and consider revising it if needed. .. We discuss new information relative to your asthma, such as new medications that may be helpful for controlling your symptoms. .. If needed, I order changes in the strength or frequency of your controller or trigger medications. .. When we have your new plan finalized, I transmit your medication refills  to the pharmacy of your choice, electronically. .. Then, we review your Asthma Action Plan and I Send to your phone! Before we sign-off, I schedule your next appointment, 3 months away. In this way, Asthmaniac helps you stay on top of your asthma. ePrescriptions * Asthma controllers are medications that “control” the underlying inflammation in your breathing tubes. * Inhaled corticosteroids are the most common type of medicine used for this purpose. * In order for corticosteroids to work, they have to be inhaled deep into the breathing tubes twice daily. * A new class of drugs has been developed to control inflammation: they are called biologics and are injected weekly or monthly into the skin of your abdomen.   Although there are a number of medications used for asthma control, the most common type is an inhaled corticosteroid (ICS). This type of steroid does not make big muscles or affect sexual function. It works by tamping down inflammation in your airways. These medications are either liquids that are sprayed from a pressurized canister into the airway or they are delivered as dry powder, inhaled using a specialized canister. They work by landing on the surface of your airways, seeping into the cells, and suppressing the run-away inflammation. There are several chemical compounds that are used including: fluticasone, beclomethasone, mometasone, flunisolide, ciclesonide, and budesonide. Each comes in several strengths and each is administered every day to keep a lid on the ongoing inflammation. When we have the dose right, you will not need your rescue inhaler more than once a month! 2020 Focused Asthma Guidelines: What they mean for You! Asthma Management Guidelines 2020 Focused Update: What’s In It For You?
Asthma Management Guidelines 2020 Focused Update: What’s In It For You? * The National Institutes of Health, National Heart, Lung, and Blood Institute coordinated this update * Recommendations were published on December 1, 2020. * It Focuses on six areas: ….Fractional Exhaled Nitric Oxide Testing ….Indoor Allergen Mitigation ….Intermittent Inhaled Corticosteroids ….Immunotherapy in the Treatment of Allergic Asthma ….Bronchial Thermoplasty * Only four of these six areas have immediate implications for asthma disease management in primary care. * I’ll review the recommendations briefly here. Major Points: These guidelines are not as wide-ranging as those developed and released in 2007. The new recommendations are based on reviews of the scientific literature and the use of the GRADE methodology for assigning a level of certainty (Low, Moderate, or High) for each recommendation. Of the six areas, only numbers 2, 3, 5, & 6 have practical implications for primary asthma care. The full guidelines:  2020 Focused Updates to the Asthma Management Guidelines:… I have put together a quick-read summary of the recommendations.  I have only included recommendations that have moderate or high certainty of benefit for patients.   Indoor Allergens: Pillow covers and mattress bags to seal in allergens are recommended if you have symptoms.  (Moderate Certainty) Allergy Shots: SCIT (Subcutaneous Immunotherapy) is recommended for adults and children (>5 yr) with mild to moderate allergic asthma as long as they have lab or skin testing data to support the allergy. (Moderate Certainty). Wheezing in Kids: Children from ages 0-4 yr with recurrent wheezing (3 x /year), triggered by colds and URI’s should have a short course (7-10 days) of inhaled corticosteroid at the onset of a new wheezing episode. (High Certainty). In ages 4+ with moderate to severe-persistent asthma, a combination inhaler containing formoterol and steroid can be used as both a daily controller and as a rescue medicine. (Moderate to High Certainty). In ages 12 + with mild persistent asthma, inhaled corticosteroids should be used daily, either along with rescue albuterol or in a combination inhaler that includes  albuterol. (Moderate Certainty). In age 12 + yr with uncontrolled, persistent asthma, Long Acting Muscarinic Antagonist (LAMA) medicine can be added to Inhaled Corticosteroid (ICS) for better control. (Moderate Certainty). Rescue Inhalers
Rescue Inhalers * Rescue inhalers are essential * The albuterol in rescue inhalers opens breathing tubes quickly * If it doesn’t quickly improve your breathing, go to the ER * A spacer should be used with all liquid medicine inhalers * Generic albuterol is now available for about $22 per inhaler  You and I both know this – your rescue inhaler is your ‘go-to’ when the wheezing starts.  Why? Because you get immediate relief (usually). HOW? This type of inhaler creates a cloud of medicine:  either dry medicine particles or medicine droplets that are carried deep into your lungs with a deep breath. When this medicine hits the inner lining of your breathing tubes (also known as bronchi), it soaks into the muscle layer around the tubes, causing them to relax.  That makes the tube get larger inside, allowing more air to move.  This makes your breathing much easier. The medicine in these inhalers is albuterol or a variant of it.  It has a short duration of action which means:  it works quickly but  It wears off quickly. For this reason, it is meant for temporary relief.  In a typical asthma action plan, albuterol is used for immediate relief or what we call RESCUE.   If it works, you are encouraged to consult with your doctor to understand what triggered that wheeze.   If we can’t be sure that you’ll be able to avoid that trigger, additional medication will be prescribed to overcome this sensitivity. There are some issues: SOMETIMES YOU DON’T GET RELIEF:  Sometimes the reaction in your lungs is too advanced for albuterol to reverse.  You will know this if you do not get breathing relief within 10 minutes of using your inhaler.  It is reasonable to try a second dose according to your Asthma Action Plan, but if you don’t get relief from that, it is best to get headed to the ER. YOU MUST USE A SPACER:   Liquid inhalers create a cloud of liquid droplets.  It is important that you use a spacer to hold those droplets, suspended as a cloud, until you can fully draw them into your lungs.  This means you must use a spacer.  If you don’t, the medicine in your inhaler simply ends up on your tongue. It will not help your breathing from there. COST:   Albuterol is expensive!  Albuterol has been put in two forms: metered dose inhaler (MDI) and  dry powder inhaler (DPI).   Both delivery systems are very good but both systems have been manufactured under band names with typical cash prices ranging from $90-$120, until now! On April 9, 2020, the Food and Drug Administration (FDA) announced approval of a generic albuterol metered dose inhaler.  It is priced at $22 per inhaler (drugs.com).  Now that’s pretty exciting when you are trying to get and stay    Serious About Asthma. Asthma Triggers * Asthma controllers are medications that “control” the underlying inflammation in your breathing tubes. * Inhaled corticosteroids are the most common type of medicine used for this purpose. * In order for corticosteroids to work, they have to be inhaled deep into the breathing tubes twice daily. * A new class of drugs has been developed to control inflammation: they are called biologics and are injected weekly or monthly into the skin of your abdomen.   Although there are a number of medications used for asthma control, the most common type is an inhaled corticosteroid (ICS). This type of steroid does not make big muscles or affect sexual function. It works by tamping down inflammation in your airways. These medications are either liquids that are sprayed from a pressurized canister into the airway or they are delivered as dry powder, inhaled using a specialized canister. They work by landing on the surface of your airways, seeping into the cells, and suppressing the run-away inflammation. There are several chemical compounds that are used including: fluticasone, beclomethasone, mometasone, flunisolide, ciclesonide, and budesonide. Each comes in several strengths and each is administered every day to keep a lid on the ongoing inflammation. When we have the dose right, you will not need your rescue inhaler more than once a month! Phone Visits *  Quality Asthma care depends on self-management by patients. *  A large part of my role as your physician is teaching you these self-management skills. *  Crucial to these skills are techniques you will use to assess: *  Your Symptoms (Asthma Control Test) *  Your Lung Function (Peak Expiratory Flow) *  We will discuss your self-assessments by phone rather than in-person. *  These phone visits and your frequent use of the Asthmaniac website are designed to keep your asthma controlled while avoiding illness exposure and the inconveience of travel and waiting rooms   I designed Asthmaniac around the National standards for asthma care. Those standards tell doctors to teach their patients how to assess their everyday symptoms and lung lung capacity. Years of research has led to the creation of a solid symptom assessment tool (Asthma Control Test or ACT).   This is a survey that you can complete yourself and then report to me. Research has also shown that lung performance - or how well you are able to breath out (Peak Expiratory Flow or PEF), can be measured using a simple, pocket-sized flow meter. In my Asthmaniac clinic, I ask that patients use a simple flowmeter to record their number several times each week. I instruct you how to use this number in conjunction with your Action Plan. I use this number in planning updates to your medications, their dosages, and your Action Plan. The great thing about having these two proven methods of assessing your asthma is that we can conduct our entire asthma appointment over the phone. Yes! Over the phone! That means you can get your Asthmaniac appointment on a lunch break, walking down the street, or wherever you are. One place you won't be is sitting? Steroids in Asthma Why do we use steroids in asthma? * Inflammation of the airways is the underlying problem in asthma * Steroids like Prednisone and Decadron are taken by mouth * Steroids like betamethasone and fluticasone are inhaled like albuterol. * Steroids are used to control inflammation in asthma * Lung inflammation in moderate asthma can be controlled but not cured.   The basic problem in asthma lungs is inflammation. Inflammation means swelling, oozing, and fragility of the breathing tube linings.   Extra mucus is produced from this inflammation and together with the swelling, creates a lot of extra resistance to air flow.   When these breathing tubes are fired up with swelling and mucus, the effort to move air, especially out of the lungs, can be exhausting. All of my work as your doctor is aimed at keeping a lid on this inflammation.  The tendency for it to get out of control will always be there – that’s the way asthma is. But, with the right medication and avoidance of triggers, you can stay out of the ER, get restful sleep, and be as active as an Olympic athlete! Now, when the inflammation is out of control, we have only one choice. SHUT IT DOWN. The most potent and fast acting medicine we have for this is corticosteroids.   We say steroids for short.   This isn’t the steroids injected by weight lifters to make big muscles - those are called anabolic steroids..   Corticosteroids work inside the cells of the breathing tube walls to block the inflammation process. They work – but their action is not immediate. Prednisone and Dexamethasone are the most common forms used in non-hospitalized asthma patients. These steroids are taken by mouth, or if your breathing is really bad, they are given intravenously. Either medicine must be taken for 3-5 days straight for the full effect. After these medicines shut down the inflammation process in the breathing tubes, we are in a position to launch a new asthma action plan that includes stronger controller medicine in the form of inhaled steroids, also called controllers. These inhaled corticosteroids have dramatically changed the course of asthma.   Lives have been saved and many days of disrupted sleep, work, & school have been prevented.  For more severe asthma sufferers, there are new medications – biologics, that are likely to make even more dramatic improvements in severe asthma control.     For most moderate asthma patients, inhaled corticosteroids will be a cornerstone of their asthma action plan. Finding the right type of steroid compound, the frequency that works, and the one that you and your insurance company accept is a really important part of asthma care. If you are using your albuterol rescue inhaler daily and aren’t using a steroid inhaler, you need one.   If you are using your albuterol rescue inhaler more than two times per week and you ARE on a steroid inhaler, you need a change that delivers more steroid either per dose or more times per day. Affordability Affordability is Important for Asthma Control * The cost of asthma control can be a barrier: * Many costs of asthma care can exceed what those without insurance can afford.  These include: * The cost of asthma medication. * The cost of doctor visits * The cost pulmonary function testing.   The cost of medical care is a big deal. People have lost their homes due to medical bills. And during this recession, it’s even worse. People in the US routinely postpone medical care because of the cost. So while I am focused on accurately diagnosing your asthma and prescribing essential medication, I can’t lose sight of the costs to you. All of my effort, logic, dedication, training and energy are completely undone if you cannot afford a consultation with me in the first place. My commitment in the design of Asthmaniac is to keep the price you pay for visits with me, fair and reasonable. I do not control the cost of asthma medicine but will do all I can to find discounted medication for my patients. So many of my patients do not have health insurance. Because of that, they have to find the best value for their medical care. That means getting the most for the least. Even patients with high-deductible health insurance have to pay out-of-pocket until their deductible is met. So they too are interested in the best value. I am committed to designing a method that asthma patients can actually use. A method they can AFFORD! A method that keeps them breathing deeply, sleeping soundly, and striding through their days with new confidence in managing their asthma.
